2. Date of Last Flight Physical

3. Date of last Form 8

2. Complete IACRA

1. Military ID

2. FTN Number (from IACRA)

3. IACRA Login & Password - written down

4. Proof of Graduation from a U.S. Military Pilot Training Program

5. Latest Form 8 (within 24 months)
